Complete the enrollment process by submitting the required forms, including:

Medical Clearance

Secure medical approval by obtaining a physician’s clearance that:

– Confirms the need for adult day care services

– Outlines specific care requirements and recommendations

This ensures we provide the best possible support for your loved one.

Personalized Care Plan

Collaborate with our care team to create a customized service plan that:

– Sets personalized goals and objectives

– Outlines tailored activities and interventions

– Identifies specific support services and accommodations

This ensures your loved one receives care that’s tailored to their unique needs, preferences, and aspirations.

The final step!

Once we’ve received all necessary documentation, finalized your loved one’s personalized care plan, and completed a successful trial visit, we’ll:

– Confirm enrollment

– Schedule a start date

Fees

Understanding Our Fee Structure:

– Medicaid waiver holders: DMAS or CSB may cover fees

– Private pay: Fees are determined by level of care and attendance frequency (number of days/week).
